Almost everyone would ultimately prefer to stay in their own homes for as long as possible providing that it is safe and manageable for those who are caring for them.

This issue normally arises when someone has been debilitated through injury, illness or after an operation and has had a hospital or convalescence stay. The ideal scenario for almost everyone is to get home as quickly as possible and get on with their day to day lives.

This can sometimes become more difficult to achieve than expected especially if there has been a significant change in an individuals’ independence and mobility levels which negatively affect their ability to perform everyday tasks.

Whether this is a short term or long term prognosis there is a considerable advantage to availing of therapy or recommendations in your own home. By being assessed in your home your therapist can provide you with rehabilitation programmes that can be carried out safely.

If the change in you or your loved ones independence is likely to be long term then the likelihood of remaining in your home is greatly enhanced by adapting your home to allow the greatest level of independence possible.

Physiotherapy

Physiotherapy involves evaluating, diagnosing and treating a range of injuries, disorders and disabilities in order to optimise the person’s mobility and functional capacity.

What Happens in a Physiotherapy Home Visit?

Quite commonly one of our physiotherapists would come to you or your loved one in your own home to carry out a comprehensive assessment of your current level of physical ability. Then a treatment plan is devised to maxmise your rehabilitation potential including a home exercise programme that is designed specifically for you. Follow up treatments and a reassessment date are then scheduled in order to measure the effectiveness of your rehabilitation. Our therapists provide weekly updates on progress to family members if required with continuous consultation in relation to the best practice and probable outcomes.

Some of the more common reasons for patients availing of these services include;

  • Post-surgical patients of knee or hip replacements
  • People who have had a fall with a significant injury
  • Clients who have suffered a stroke
  • Neurological conditions such as Multiple Sclerosis or Parkinson’s disease
  • People who have generally deteriorated due to a long illness or hospital stay

 

Occupational Therapy

Occupational Therapy is designed to promote health, prevent injury or disability and develop, improve or sustain an individual’s level of independence.

What Happens in a Occupational Therapy Home Visit?

Our Occupational Therapist would carry out an assessment in your place of proposed residence initially taking into account any recent illness or injury. They would ensure that your living space was as safe as possible while enabling you to remain as independent as possible.

In addition our therapist would assess you and any current specialized equipment (chairs, splints) to ascertain if they are suitable to sustain or improve your level of independence.

A detailed report would then be provided sometimes recommending alterations to your living space (e.g grab rails, ramps etc) or a recommendation for specialised equipment or furniture. A follow up trial of equipment would often be arranged.

 

Falls Risk Assessments

This type of assessment is normally carried out after an individual has had a fall or multiple falls whereby the exact cause or causes of the falls may be multifactorial.

We initially come to your home, or you visit our Fit For Life clinic (sometimes after a referral) and carry out a comprehensive falls risk assessment. You will complete a battery of tests to identify functional, medical and psychological risk factors.

Following our initial assessment we typically would
carry out a full assessment of your home highlighting any changes that could be made to reduce the risk of further falls.

We go through our findings, giving you time to ask questions to clarify any of the results. A report is provided with recommendations to support you with maintaining your quality of life. You may have new interventions introduced and support pathways are explored and offered to you.

 

Exercise Therapy

Fit For Life provide exercise and rehabilitation therapists for those individuals who have been prescribed a home exercise programme but need or want assistance in order to carry it out correctly.

Our therapists are also available to come and assess your current ability in carrying out activities of daily living and design and instruct you on the most effective way to increase your physical activity levels.

 

Carer Training

We provide extensive training for carers that work looking after their family members and carers employed by public and private healthcare companies.

If you are caring for someone in their own home there are lots of areas we can support you in. With our practical training workshops you will be able to put what you have learned into your day to day practices.

This training provides safe yet time efficient methods and strategies for assisting the person you are caring for to maximise their independence and dignity. All this is done while ensuring that both of you are safe and at a reduced risk of injury.

Our team of experts have developed workshops in areas such as;

  • People Manual handling – Bed, chair, bathroom
  • Falls Prevention
  • Safe transfers and mobility (see specialised training section page 47)
  • Strategies for people with behavior that is challenging
     

Specialised Healthcare Equipment Consultation

Following any Fit For Life assessments carried out by one our of our therapists or a referral or recommendation from another healthcare professional, Fit For Life can provide further additional support and expertise in implementing any action plans.

Our therapists can provide specialist advice and expertise in recommending and sourcing the appropriate specialist equipment and/or furniture and fittings to enable an individual to remain living at home as independently as possible.

In order to maxmise an individual’s opportunity to remain as independent and/or comfortable as possible, it is very important to choose the correct equipment/devices or furniture for the individual themselves but also those caring for them.

Common areas where we assist individuals include:

  • Bed and mattress prescription – extremely important for enabling sleep patterns but also vital in promoting bed mobility and ease of transfer in and out of bed if required
  • Manual handling equipment – Bed sheet systems, Hoists
  • Chairs, Wheelchairs, Specialised wheelchairs
  • Bathroom adaptations
